Trainee Registration

To register, complete the form below. In order to complete your sign up, follow these instructions properly:

1. Username: Your must be only 4 characters:  EX: CDTZ for Craig Dietz
2. Email: Use your work email or acceptable personal email account
3. Password: Your must be 8 characters: 1 special symbol (!$*#); 1 Capital letter: EX: !Craig15
4. First Name: Your first name
5. Last Name: Your last name
6. Membership Level: All Trainees by default are Free
 
Make sure you click the “Register” button or else your form
will not be submitted. If you do not receive an email or if you have any difficulty,
contact your supervisor or trainer for assistance.
